Advancing Colorado's Mental Health Care
2005 to 2011

A joint effort of The Trust, Colorado Health Foundation, Caring for Colorado Foundation and The Denver Foundation to support human services agencies, mental health care providers and others to improve the integration and coordination of mental health services with physical health care so that people can be treated with the services they need most, regardless of where they seek care. 2005 to 2012

Increased the ability of organizations and educational institutions to provide culturally appropriate health care services to help reduce racial and ethnic health disparities in Colorado. Learn more.

Partnerships for Health
2005 to 2012

Improved the coordination of health services at the community level through partnerships that build, strengthen and sustain the infrastructure of Colorado communities. Learn more.

Healthy Aging - Evaluation
2006 to 2011

An independent evaluation was conducted by the National Research Center, Inc. to determine whether providing technical assistance to build the capacity of grantee organizations helped to improve their ability to serve seniors. Grantees reported program-level outcomes, and the evaluation examined associations between these outcomes and increased organizational capacity.

Homelessness Prevention
2006 to 2010

Provided support for the administration of the Homeless Prevention Tax, and committed funding to support Denver’s Road Home, a national model for helping to end homelessness.

Colorado Department of Health Care Policy and Financing (HCPF) – System Improvements
2007 to 2011

Funding to strengthen HCPF staff, planning, outreach and public education efforts to support the development and implementation of health care system improvements in Colorado. As well, through the Colorado Governor's Building Blocks to Health Care Reform and Executive Order D 005 08, the establishment of the Center for Improving Value in Health Care (CIVHC) to identify and pursue strategies for quality improvement, cost containment and improved population health.

Office of the Colorado Governor
2007 to 2012

Supported a staff position in the Colorado Governor's Policy Office to assist with the implementation of laws that will increase access to health for all Coloradans.
