Overview

Integrated, coordinated and efficient health systems can significantly improve the delivery of  timely, cost-effective, quality care that produces positive health outcomes. It is especially important for children to have a “medical home” where they can receive continuous primary care that includes family involvement and ensures that they receive preventive health care. Yet 180,000 Colorado children do not have a usual source of care, even though they are eligible for, or enrolled in Medicaid or Child Health Plan Plus (CHP+). Additional approaches to improving the health system include coordinating the delivery of health care services across multiple providers and health care settings – primary, specialty, behavioral and oral care; aligning financial incentives that reward providers for achieving improved health outcomes; and putting in place a  health information technology system to improve quality and safety.

Grant support to improve and strengthen health systems in Colorado includes:

  • Learning how to better align high quality, coordinated care with financial incentives through a medical home pilot
  • Making comprehensive preventive services for children and youth available in a timely manner
  • Ensuring that health care sites and providers meet safe and high quality standards of care
  • Identifying strategies to provide quality health care while controlling costs and eliminating duplication of services.
